Bootcamp Signup

I, the undersigned participant, hereby acknowledge and agree to the terms and conditions set forth in this liability waiver (“Waiver”) for participating in the bootcamp session at Nacho Shack (“Restaurant”). By signing this Waiver, I understand and accept that I am solely responsible for my actions and well-being during the bootcamp session.

Assumption of Risk: I understand that participating in physical activities during the bootcamp session involves certain inherent risks, including but not limited to muscle strains, sprains, falls, and other injuries. I voluntarily assume all such risks associated with my participation.

Release of Liability: In consideration of being allowed to participate in the bootcamp session at the Restaurant, I hereby release and discharge Nacho Shack, its owners, employees, contractors, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during or as a result of my participation in the bootcamp session.

Indemnification: I agree to indemnify and hold harmless Nacho Shack, its owners, employees, contractors, and representatives from any and all costs, expenses, damages, or liabilities, including attorney fees, arising out of or related to any loss, damage, or injury caused by my actions or participation during the bootcamp session.

Medical Clearance: I represent that I am physically and mentally fit to participate in the bootcamp session. I acknowledge that it is my responsibility to consult with a healthcare professional regarding my physical condition and ability to engage in physical activities.

Personal Belongings: I understand that Nacho Shack is not responsible for the loss, theft, or damage of any personal belongings during the bootcamp session. I agree to solely be responsible for the security and safety of my personal belongings.

Severability: If any provision of this Waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

Governing Law and Jurisdiction: This Waiver shall be governed by and construed in accordance with the laws of the [State/Country]. Any legal action or proceeding arising out of or related to this Waiver shall be exclusively venued in the courts of [State/Country].

I have read this liability waiver carefully and fully understand its contents. I am aware that by signing this document, I am giving up certain legal rights and remedies, including the right to sue Nacho Shack for any injuries or damages sustained during the bootcamp session. I voluntarily sign this liability waiver without any inducement, assurance, or guarantee.
